The Registered Nurse (RN) provides skilled nursing services to patients in their homes in accordance with physician orders, the individualized Plan of Care, agency policies, and all applicable Rhode Island Department of Health and federal regulations.
Responsibilities
- Complete comprehensive nursing assessments
- Develop, implement, and update individualized Plans of Care
- Administer medications, injections, wound care, and other skilled nursing procedures
- Monitor patient progress and document all care provided
- Educate patients and caregivers on disease management and medications
- Coordinate care with physicians and other healthcare providers
- Participate in Quality Assurance and Performance Improvement (QAPI) activities
- Respond to patient emergencies as needed
- Maintain accurate and timely electronic documentation
Qualifications
- Current Rhode Island RN License
- CPR Certification
- Minimum one year of clinical nursing experience (home care preferred)
- Valid driver’s license and reliable transportation
- Excellent communication and organizational skills
