Duties of a Skilled Nurse (LPN/RN)
- Comprehensive nursing assessments and individualized care plans
- Medication administration, management, and education
- IV therapy and infusion services
- Wound care, dressing changes, and post-surgical recovery care
- Chronic disease management — diabetes, heart failure, COPD, hypertension
- Catheter, ostomy, tracheostomy, and feeding tube care
- Respiratory assessments and oxygen therapy management
- Vital signs monitoring, blood draws, and specimen collection
- Pain management and end-of-life / palliative care support
- Patient and family education on diagnoses, treatments, and disease management
- Care coordination with physicians, therapists, and pharmacies
- Home safety and fall-risk evaluations, with emergency assessment when needed
- Support for rehabilitation and recovery following illness, injury, or hospitalization

What to Expect
A care coordinator will conduct an initial assessment and create a personalized care plan. Visits are scheduled based on physician orders and your specific needs, with 24/7 on-call nurse support between visits.
